Each year, more than 640,000 individuals are released from state and federal prisons, and in  California, nearly half are expected to return within three years. For those living with mental health  challenges, the risk of returning to incarceration is even higher. These individuals often face deep rooted trauma, systemic barriers, and a lack of access to appropriate care.

Together Let’s Rise,  we understand the complexity behind recidivism, and we’re here to break that cycle through  compassion, connection, and comprehensive care.

Our programs are designed with the understanding that healing and transformation are possible  when people are met with empathy and supported through every stage of their journey. From re entry to youth mentorship, mental health support to workforce development, every offering we  provide is rooted in love, justice, and opportunity.

Forensic Mental Health Program

Our Forensic Mental Health Program offers a holistic path to healing and reintegration for system impacted individuals. We focus on prevention, diversion, and reentry, blending trauma-informed  care, culturally specific services, and social support. Every participant is treated with dignity and  empowered to reclaim their future. This isn’t just a program, it’s a fresh start grounded in  community, safety, and hope.

Youth Development & Gang Prevention Program

We believe that every young person deserves the chance to grow beyond their circumstances. Our  gang prevention work uses a client-centered, therapeutic approach that helps youth identify and  manage anger, process trauma, and understand the reasons behind gang involvement. We work  to interrupt the cycle before it becomes a life sentence, by giving youth the tools to heal, reflect,  and rise into leadership. ​

Project W.A.S.H. (We Are Still Here)

This mentorship program serves youth ages 14–17 who have experienced incarceration  themselves, have an incarcerated parent, or are at risk of either. Through consistent, one-on-one  mentorship, we help these young people feel seen, supported, and valued. Project W.A.S.H.  reminds them they are not forgotten, they are still here, and they still matter. ​​

Project Empowering Futures

For unhoused youth navigating employment barriers, we offer more than just job training, we  offer empowerment. This program provides vocational training, job readiness workshops, resume support, mentorship, and connections to internships or employment opportunities. With each  step, we help youth build self-confidence, purpose, and long-term economic stability.

Project R.O.W ( Recognizing Our Worth)

Project R.O.W. serves young adults ages 18–25 who are unhoused and impacted by the justice  system. This mentorship-based program creates space for participants to express themselves, be  heard, and rediscover their worth. At the core of R.O.W. is the belief that every young person is  the captain of their own destiny. All they need is encouragement, and the support to grab the oars  and row toward their future. ​​

Project Harmony

Family reunification takes more than time, it takes intentional healing. Project Harmony is a court approved parenting and counseling program focused on restoring the bond between parent and  child. Through family counseling, communication training, mindfulness, and anger management  tools, we help families reconnect with empathy and love, rebuilding trust one step at a time. ​

Project Change Direction

Chronic stress can take a serious toll on both body and mind. Project Change Direction offers a  healing space where individuals learn to recognize the physical, emotional, and behavioral impacts  of long-term stress. We teach evidence-based strategies like mindfulness and cognitive-behavioral  techniques to help participants regain control of their mental and emotional well-being. ​

Intensive Case Management

Our case management services offer personalized, ongoing support tailored to each client’s  unique challenges. We help individuals access physical and mental health care, substance use  treatment, and housing support. Through comprehensive assessments and service planning, we  guide clients toward long-term stability with compassion at every turn. ​

Behavioral Health

Mental health and substance use struggles don’t have to be faced alone. We provide culturally  responsive behavioral health services that honor each client’s background and experiences.  Whether someone is navigating trauma, addiction, depression, or anxiety, we meet them with  empathy and clinical care, walking with them every step of the way.​
